I understand that data collected during the study may be looked at by researchers from the University of Southampton and Reveal My Food and used to inform the impact of such food ordering apps on the quality of the diet of employees.

I consent for data about my use of the Reveal My Food intervention to be collected by Reveal My Food and passed on to the research team. Any personal or financial information will not be passed on.

I understand I can withdraw at any time without giving reasons and that I will not be penalised for withdrawing nor will I be questioned on why I have withdrawn.

I agree to share the results of the latest blood work with Reveal My Food for the purpose of the study.
